These ar to Certifie whom it may concern that the bearer hereof Captaine Joseph Smyth was borne in the parish of Shalbourn in the County of Wilts[hire] & hath since liued in Malborough w[i]th Mr Walter Bayly till a little before these warres & hath euer since these troubles binn a seruant to the Commonwealth of England & hath in the seruice thereof lost the vse of his limbes & hath had [illegible] no other setled beeinge since but in this County
