The humble petic[i]on of Thomas Dauis of Newbury Streete in the p[ar]ish of Preshut in the county aforsaid weauer

Vnto yo[u]r Wor[shi]pps that yo[u]r pet[itione]r serued (his late Ma[jes]ties of euer blessed memory) in the late vnhappie warrs vnder the Comaund of that truly valiant and hon[ou]r[a]ble Personage ye right hon[ou]r[a]ble the Lord Hopton in the Regim[en]t of horse of S[i]r Edward Stawells in Maior Edmund Bowers Troope as Corporall And in the seruice at seuerall fights (vizt) At Taunton Bridgwater Lansdowne ye Deuizes Roundway the last Newbury fight at Cropredy Bridge Banbury the routing of the Earle of Essex in Cornewall yo[u]r pet[itione]r receiued seuerall wounds but espetially yo[u]r pet[itione]r rec[eive]d an vnfortunate wound in his right arme w[hi]ch hath totally disabled yo[u]r pet[itione]r at his trade soe that yo[u]r pet[itione]r is not in the least measuer able to worke thereby to gaine anything towards a subsistance for himselfe his wife and six smale children: Now since his Ma[jes]ties happy restaurac[i]on to his crowne and Kingdomes.
Yo[u]r pet[itione]r doe therefore humbly Implore yo[u]r wor[shi]pps to giue credence to the Certificate herevnto an[n]exed And conferre such an Annuall Pencheon on yo[u]r poore pet[itione]r as may refresh the bowells of his wife and family.
