To the Right Wor[shi]ppfull the Justices of the Peace assembled att the Gen[er]all Quarter Sessions of the Peace held att New Sa[rum] for the county of Wiltes[hire]

The humble petic[i]on of Roger Hellaker of Horningsham in the county foresaid husbandman

<Sheweth>

That yo[u]r poore Petic[i]oner served his late Ma[jes]tie of ever blessed memory dureinge all his warrs & hath received soe many wounds in his body in the said service that hee is vtterly disabled to mayneteyne himselfe & family as by a Certificate herevnto annexed may appeare

Yo[u]r Petic[i]oner therefore humbly prayeth yo[u]r Wor[shi]pps to p[er]vse yo[u]r Petic[i]oners certificate & to consider of his sad condic[i]on & allowe yo[u]r petic[i]oner such an yeerely penc[i]on as yo[u]r Wor[shi]pps shall thinke fitt And yo[u]r Petic[i]oner shall dayly pray &c
