To the Right hon[oura]ble the Justices of peace for the County of Stafford

The Humble petic[i]on of John Dale of Burntwood in the County afors[ai]d maimed soldier, is that yo[u]r Worships will be pleased to Allowe hime some yearly meinteynance, as you in yo[u]r Wisdoms doe thinck meet, I hee haueing lost the vse of his left arme in the stats service [illegible] & hath not as yet receiued any Benifitt for the same out or from the treasurers of this County for maymed souldiers.

And yo[u]r pet[itione]r will ever pray &c.
