These ar to sertyfye your Worships yt William Samford was first shoot thorow the leg at Borowshout by Axmester in the parlament service under the old Captaine Pine & ever sence hath had a running issue there.

Sence he was under Collonell Humes for the Committee & in his service was shott with a brace of bulletts under the short ribes whereof which went downe ward and th[w]art the buticke of which hee was cured till this tyme: now it doth again truble him as we cann witness
