To the Honor[a]ble Com[m]ittee for the Army:

These are to Certifie yo[u]r hono[u]rs That Edward Taylor served as Trooper of my Troope in the Regim[en]t of Col[one]l Hortons And was maymed in his left Arme in North wales att the Routt of S[i]r Henry Lyngen and Receaved alsoe att that tyme dyvers several wounds. And now disabled to doe any further service. Wherevpon I discharged hym. Desiringe yo[u]r hono[u]rs to looke vpon his sadd Condic[i]on. And take it into yo[u]r Considerac[i]ons for his releif. And I doe hereby further Certifie yo[u]r hono[u]rs That he in the said service behaved hymself faithfully and Valyantly like a Souldier. And soe I held my selfe obliged to attest the truth thereof And soe leave it to yo[u]r hono[u]rs Considerac[i]on:

Yo[u]r hono[u]rs devoted servant.
