To the Right hono[ura]ble ye Committee of Lords & Com[m]ons for the Army

These are humbly to Certifie That the bearer hereof Samuell Winsor was in Actuall seruice for the Parliament, and was wounded & maymed in his hand and Legg haueinge lost the use of {three illegible words} fingers beinge vnder my Com[m]and in Colonell Okeyes Regiment by reason whereof hee hath not the use of his hand as formerly hee had w[hi]ch can[n]ot chuse but be a greate hindrance to him in his Callinge
