Theise are to Certifie whom It may Concerne that the bearer hereof Richard Jacob was A Trooper in the late warr for the service of the State, vnder my Comaund in the Regim[en]t of Collonell Edmond Ludlow; And that he behaued himselfe honestly faithfully & stoutly And that he the s[ai]d Richard Jacob Received such dangerous wounds in the s[ai]d s[er]vice, As haue made him vncable [sic.] of any further service. In testimony whereof I haue heerevnto sett my hande & seale ye day & yeere afores[ai]d.

That the s[ai]d Richard Jacob was dandgerously woonded in the Par[liam]ent Seruis is attested by me
