These are to certifie ye hono[ra]ble Bench that I have view[e]d ye body of Phillip Warren of Bradninch, on whome I finde ye effects of divers greate wounds formerly received, whereby I conceive the said Phillip Warren is now rendr[e]d not a little uncapable to get his living by reason of ye present debilitie of these wounded parts.
