To ye Justices of ye peace for ye Com[m]on welth of England assembled at ye quarter sessions at Warmester

Wee whos names are herevnto subscribed doe Certify that that [sic.] the bearer hereof Jeremy Ballard of Bromham Thatcher is one w[hi]ch for the space of a yeare & vpwards did serue the p[ar]liament vnder the Command of Collonell Tho[mas] Eyres vntill such tim as ye seige of ye Devizes hee was hurt by a great shott by reason wherof hee was Constrained to lye at surgery nine month & no waye able to laboure for his livinge; to his great hindrance hee is one that never swerved fro[m] ye p[ar]liament[ar]y seruice but Continued ther in as longe as hee was able hee hath not receiued any recompence nor binn Chargable to any for ought that wee knowe during his inabillity to laboure
