The certificate for Jeremy Ballard of Bromham, Wiltshire, 23 June 1649

These are to Certify all whom it may Concerne that this bearer Jeremy Ballard served as a foote Souldier in my Company in ye Regim[en]t of Col[onel] Nathaniel Fiennes Governour of Bristoll; & when the Devizes was beseiged by S[i]r W[illia]m Waller hee received a shott w[i]th a Cannon Bullett whereby hee was disabled to maintaine himselfe by his labour but lay a long time vnder the Chirurgeons hands vpon his owne charge & have not received any releife, by w[hi]ch meanes hee is much impoverished & indebted. In witnes whereof I have herevnto set my hand this 23th of June 1649.
