To the Right Wor[shi]pfull the Judges of the Sessions and the rest of ye wor[ship]full bench.
The humble petic[i]on of John Coombe of Bradninch in the County of Devon
<Sheweth>
That yo[u]r orator being in his Ma[jes]ties service of blessed memory, under ye command of the right honor[a]ble the Lord Berkley, S[i]r Rich[ard] Chamley, Capt[ain] Sainthill and others dureinge the time of the late warrs in w[hi]ch said service yo[u]r petic[i]on[e]r received Severall wounds and shots (as by a certificate hereunto annexed appeareth), whereby yo[u]r petic[i]on[e]r is forever disinabled to labour at his calling for the maintainance of himself and his family: Now soe [it] may please your wor[shi]p[s] that yo[u]r petic[i]on[e]r never receaved any relief but hath for many yeares last past endured much torment, paine and misere, and is at this time in a very lowe and miserable condic[i]on.
The premisses considered and for that yo[u]r petic[i]on[e]r hath a wife and three children to maintaine, for whose subsistance yo[u]r petic[i]on[e]r is not able to labour by reason of the continuall paines hee lays und[e]r occasioned only by the said wounds, yo[u]r petic[i]on[e]rs most humble request is that you would please to consider his sadd and deplorable condic[i]on, and allow him some thing towards the maintainance of him and his family.
And yo[u]r petic[i]on[e]r as in duty bound shall pray &c
