The humble petic[i]on of Henrie Cottom of Chippinge

<Sheweth>

That yo[u]r petic[i]oner hath served the Comonwealth in England and Scotland and never revolted And beinge fallen lame through cold fallen vpon him in the said service and vnable to labour for his maintenance as heretofore is in danger to starue vnles in due tyme releeued

Humblie prayeth such Monthlie or other allowance for his maintenance as A Maimed souldier as in yo[u]r Wor[shi]pps discretion may seeme meete & competent And hee shall pray &c.
