Humbly sheweth that wheras yo[u]r petic[i]on[e]rs husband beinge vnd[e]r the Comand of Col[one]l Hutlstan Phillipson & in his Maj[es]t[y]s seruice at Burton mortally wounded & ther vpon dyed & leaueinge yo[u]r petic[i]on[e]r a Charge of small Children

May it therfor please yo[u]r wor[shi]pps to take the p[re]misses into consid[e]rac[i]on & grant yo[u]r petic[i]on[e]r an allowance out of the maimed soulder money for former seruices or as in discrec[i]on you shall think requisite wherby yo[u]r petic[i]on[e]r may haue some releefe & yo[u]r peticon[e]r shall prey &c
