To the right wor[shipfu]ll the Justices of the peace for thes present quarter Sess[ion]s

Wee whose names are subscribed doe Certify yo[u]r wor[shi]pps, That the bearer hereof Mr William Evans hath bynn a liftenant Vnder Capten Goodman in the late Kings Sacred maiestys s[er]vice & in the said s[er]vice hath had severa{l} wounds soe that hee was in danger of his life, and hath spent the most parte of his estate in Cureing of himselfe & of his said wounds In testimony whereof wee have hereunto sett o[u]r hands the Twentieth day of Aprill 1644.
