These arr to Certify all whome it doth concerne that the bearer hearof Willi[am] Balhatchett of Otterton in the County of Deuon hath rec[eive]d severall wounds in his body by w[hi]ch hee is Maymed & is alltogather made vncapable of Gaineinge his Maintence by meanse theirof, witness my hand the day & yeare aboue writen
