To the hono[ura]ble Comittee of the Army

May it please your hono[u]rs These are humblie to certifie vnto yo[u]r hono[u]rs That John Chaddocke served the state as a Trooper in my troope in Collonell Hortons Regiment at the late seige of Pembrooke Castle and in that service hee was very dangerously hurt and maimed in his left arme and receaved many other wounds in his body and behaued himselfe valiantly All which I am obliged to attest and certifie to yo[u]r hono[u]rs and desiring that some care may had of him, and some speedie course taken to supplie him, I humblie take leave and rest
