These are to Certifie that the bearer hereof Thomas Salusbury did truely & faithfully serve his late ma[jes]tie of euer Blesed memorie in the Regim[en]t of Col[one]l William Salusbury as A Sergant in the Company of Capt[ain] John Salusbury & hath euer since Continued in his Ma[jes]ty service.
Wittnes my hand dated the 20th day of April 1666.
