The certificate for Thomas Mayat of Leek, Staffordshire, 4 October 1656

John Nyckson of Heaton in ye p[ari]sh of Leeke & Count[y] of Staff[ord]s[hire] yeoman testifieth that himself & Ensigne Mayat w[i]th many others being called vpon service ag[ain]st ye Enemie to Hartingto[n] were taken prisoners, wheree [sic.] ye s[ai]d Ensigne was sore wounded & maymed both in ye head & body, w[i]th sworde guns & Poleaxe, & being stript was left for dead in ye highway where he lost more blood, then any one prisoner that liued of [?174,] & being driven prisoner w[i]th the[m] towards Chesterfield on ye East moore was strook to ye ground, & at Brampton Church was againe taken up for dead by Mr Raph Ashinhurste & laid before ye fire in ye [?Bakhouse] of ye s[ai]d church, & driven to Chesterfield where we remained sixe weekes in great mise{ry} during w[hi]ch time I dressed his wounds there, & at Leek should have had his left Arme cut of as Incurable by ye advice of ye Lo[rd] Fairfax his Chrurgions, all w[hi]ch I am ready to depose upon {illegible} when I shal be called therevnto as witnesse my hand this fourth day of Oct[o]ber 1656
