I doe hereby Certifie That Brinnesley Lewis gent[leman] Actually and faithfully served in the Parliam[en]t service from the begining  of theis Warrs first as one of his Ex[cellen]cies the late Earle of Essex his Life guard vnder the Com[m]and of S[i]r Phillipp Stapleton Knight, and afterwards as Cornett to my owne Troope of Horse as well vnder the said late Lord Generall, as also vnder this pr[e]sent Generall the Lord Fairfax, vntill about the Tenth day of July 1645 att Lamport fight the said Brinnesley Lewis received many wounds whereof he [illegible] immediately  dyed. And I doe hereby further Certifie that the said Brinnesley Lewis (whilst he lived) dureing all th{e} tyme of {illegible}
