These certifie that phillip Luckham of Thurlestone in Devon was A souldier long and only in his late M[ajes]tys service vnder the late H[onou]r[a]ble Colonell S[i]r Edmund Fortescue knight his Reigment and therein continued till he was maimed in the said service and soe dismissed by his Cap[tain] Nic[holas] Reynell came home maimed and soe continues to this day all which is vpon the knowledge of
