These are to certify that the bearer hereof Mathew Foulk of the p[ar]ish of Llanvaire Talhajarne in the County of Denbigh did truely & faithfully serve his [illegible] late ma[jes]ty Kyng Charles the first of famous memory in the regim[en]t of Col[one]l Hugh Wyn & in my company & continued all along loyall to his s[ai]d Ma[jes]ty & his Ma[jes]ty {that n}ow is as witnesse my ha{nd th}is 5th day of Octob[e]r 1675.
