I Doe hereby Certifie That Thomas Mosley of Morton in the County of Derbie yeoman, was listed A foot Souldier vnder my Comand the Second Daye of December 1643, in the Garrison of Nottingham And Soe Contyned vntill the Sixt{h} Daye of Maye 1644, And then listed A troop[er] vnder my Comand and Soe remained vntill the 19th of Aprill 1645, where hee was slaine the fort att the Bridges being taken, All w[hi]ch time hee did faithfull Service for the state: In Testimony whereof I haue Sett my hand the [?8] Daye of March: Anno D[o]m[ini] 1647.
