Theise are to certifie whome it may concerne that Thomas John Lewys of the Parrish of Llanrust in the Countie of Denbigh, was in his late Ma[jes]tys service in the Regiment of Collonoll Garrett, and vnder the com[m]and of Captayne Windibanck, and was in severall Battailes and engagements, vizt: Edghill, Newberye, Braynford and other places where he receiued severall wounds, lost his hearing and is very much decayed in his health, The truth hereof I Rich[ar]d Lloyd Lievftenant then to the said Captayne doe testifie vnder my hand the Eleaventh day of July 1664: And withall desire that the bearer hereof ye aforesaid Thomas John Lewis may be admitted Penc[i]on[e]r. And he will ever pray &c.
