In pursuance of an acte of Parliament, entitled an acte for the releefe of the poore Lame, and Indigent officers, and Sould[ie]rs, who have Served his Ma[jes]tie and his Royall Father, of Famous memorie, in the Late Warrs
wee whose names are hereunto subscribed doe make bould to c[e]rtifie, his Ma[jes]ties Justices of the Peace, & Quorum, and others, whom it May concerne, that John Hilton off Wheelton, w[i]thin the Countie of Lanc[aste]r hath beene A Souldier under the Comand of Collonell Tyldesley, at Liuerpoole, and att Yorke Battle and there received Eighteene wounds, sixe in his head & face [illegible] in his Armes, and hands, Nyne wounds, one wound in his syde, another in his Thigh & anoth[e]r in his Leg, And did Thereupon Lye und[e]r the Surgeans hands, one whole yeare, The Cost and Charge whereof, did much Impoverish the poore man, And now not beeing able to Labour and beeing in declining yeares, desires to have reliefe off the sayd pay, and pention, According as others doe, and have received as his Ma[jes]ties Justices, May thinke Fit: Thus Farre wee made bould to c[er]tifie and Humbly Leave
