These are to certifie that John Coombe of Bradninch was shot [and] by w[hi]ch fractured above his back to his greate p[re]judice & will be to his dyinge daye besides severall other hurts in his body received in his Ma[jes]t[ie]s s[er]vice w[hi]ch will be testified by:
