<Burrough of Malmesbury }>

<in the County of Wiltes[hire] s[ession]s }>

To the Right Wor[shipfu]ll the Justices of the Peace of the County aforesaid

Wee whose names are herevnder written Inhabitants of ye Burrough aforesaid doe hereby Certifie that the bearer hereof John Ball late of Westport in the said Burrough hath serued the kings Ma[jes]tie that now is & his late Father in the Warres in England to our certaine knowledg & Elsewhere as wee are Crediblie informed for ye space of Twenty six yeares last past & now is by one order dismissed & accquitted ye said service haueing receiued many hurtes & woundes in his body.

Wee therefore doe most humbly desire yo[u]r Wor[shi]pps to graunte & allowe vnto him the said John Ball such Pension & allowance for his reliefe & mainteinance as is vsuall to be allowed to his Ma[jes]t[y’]s maimed Souldiers

And wee shall ever pray

<I doe remember the bearer hereof in his late Ma[jes]t[y’]s Armie A souldier, haue seene his hurts & belieue this Certificate to be true>
