<Burrough of Malmesbury }>
<in the County of Wiltes[hire] s[ession]s }>
To the Right Wor[shipfu]ll the Justices of the Peace of the County aforesaid
Wee whose names are herevnder written Inhabitants of ye Burrough aforesaid doe hereby Certifie that the bearer hereof John Ball late of Westport in the said Burrough hath serued the kings Ma[jes]tie that now is & his late Father in the Warres in England to our certaine knowledg & Elsewhere as wee are Crediblie informed for ye space of Twenty six yeares last past & now is by one order dismissed & accquitted ye said service haueing receiued many hurtes & woundes in his body.
Wee therefore doe most humbly desire yo[u]r Wor[shi]pps to graunte & allowe vnto him the said John Ball such Pension & allowance for his reliefe & mainteinance as is vsuall to be allowed to his Ma[jes]t[y’]s maimed Souldiers
And wee shall ever pray
<I doe remember the bearer hereof in his late Ma[jes]t[y’]s Armie A souldier, haue seene his hurts & belieue this Certificate to be true>

To the Right Worshipful the Justices of the Peace of the county aforesaid.
We whose names are hereunder written, inhabitants of the borough aforesaid, do hereby certify that the bearer hereof, John Ball late of Westport in the said borough, has served the King’s Majesty that now is and his late father in the wars in England (to our certain knowledge) and elsewhere (as we are credibly informed) for the space of twenty-six years last past. And now, he is by one order dismissed and acquitted the said service, having received many hurts and wounds in his body.
We therefore do most humbly desire your Worships to grant and allow unto him, the said John Ball, such pension and allowance for his relief and maintenance as is usual to be allowed to his Majesty’s maimed soldiers.
And we shall ever pray.
<I do remember the bearer hereof in his late Majesty’s army as a soldier, have seen his hurts and believe this certificate to be true.>
