These ar to Certifye that the bearer heareof Henry Nicholes of Totness hath accordinge as his peticion doth mention, lost the sight of his lefte eye, & the other is very weake, hee hath alsoe had sume woonds in his lefte hand, by reson wharof hee is not able to gaine his liuelyhood as formerly hee hath done as witness my hand this 27th day of Aprill 1663
