To the right Wor[shipfu]ll his Ma[jes]ties Justices of the Peace at the generall Sessions holden at the Deuizes for the County of Wilts[hire]
Wee whose names are herevnto subscribed doe humbly certifie on the behalfe of George Clarke Pinmaker of Marlebrough in the county of Wilts[hire], that hee hath serued his late Maiesty (of euer blessed memory) vnder the Com[m]aund of S[i]r Lewis Diues in the Company of Collo[nel] Sheirleigh vnder whose Com[m]aund hee was wounded in the hand; And afterward serued vnder the Com[m]aund of that truly honor’d the Lord Cleeueland vnder whose comaunds hee was shoot [sic.] through the Foote And lastly serued vnder the Com[m]aund of one Capt[ain]: John Parsons vntill that Infortunate tyme in ye West, By those vnhappy woundes receiued Wee humble certifie further that hee is vtterly vncapeable to mayneteyne himselfe and family, vnlesse your Wor[shi]ps please to looke one him with an eye of Pitty and Compassion and afford him such a Pencheon that may conduce to the supportac[i]on of him & his family In testimony whereof wee subscribed of Names
