To the Right Worsh[i]pp[fu]ll his Ma[jes]ti[e]s Justises of the Peace for the County of Wilts[hire] sitting at New Sarum

Wee whose names are hearevnto Subscribed doe Certifie that David Slugg of the p[ar]ish of Westbury in the County Afores[ai]d Fuller did for A long tyme serue his late Ma[jes]tie King Charles the first of Glorious memory As A soldier in the Late warres & did serue his s[ai]d Ma[jes]tie very faythfully as A sargent in his armey Army hee first tooke vpp Armes at Malmesbury in the County Afores[ai]d & served vnder Joseph Bamfield then one of his Ma[jes]ties Collonels And neuer was on the other side And that At the sige before Lime the sayd Dauid Slugg Rec[ei]ued a greiuous wounds on his Right Arme as also on his hand so that hee is thareby very much disabled to worke at his s[ai]d trade to earne mayntenance for his himselfe his poore wife & Children All w[hi]ch wee Leaue to your Worshipps & Rest y[ou]r servants
